Use the official lookup desk

You have a clinician name, a pharmacy name, and a product question. Here is where each one goes.

One search cannot answer every question. Each government office keeps its own records: the Department of Consumer Affairs for clinicians, the Board of Pharmacy for pharmacies, and the FDA for problems with a medicine.

Clinician record

California’s Department of Consumer Affairs search covers the health-professional licence types used in this guide. Search the exact name and open the full record.

Pharmacy record

The Board of Pharmacy search includes pharmacy and sterile-compounding licence types. The board also accepts pharmacy complaints.

Product problem

Patients can send a medicine problem to FDA MedWatch. Your report helps the FDA watch for safety problems across many people. One report does not by itself prove what caused the problem. For help with your own symptoms, contact your clinician.

These California and federal routes were checked as of September 2026.
